Here is the link to the most current curriculum forms for you to download, complete and submit to your Division Dean, who will then forward to the Curriculum Committee for approval. These forms are in MicroSoft Excel format. You must have Excel installed on your computer.

• Form A: Course Outline - for new course proposals
